Avenue D Services/Career Pathways assists individuals with determining appropriate Career Pathways and pursuing their goals. ADS/Career Pathways Core Operating Values Mission Statement: Fundamental to ADS/Career Pathways’ beliefs is the importance of providing all disadvantaged and/or differently-abled people with equal access to the same opportunities that all of us enjoy.

ADSCP assists said individuals by utilizing an open-minded, person-centered methodology to help all clients access opportunities such as career skills work, gainful employment, the ability to set and work toward personal life-affirming goals, finding and building a sense of community by the formation of friendships and positive peer relationships, and to learn to use any number of available resources that may enrich their lives in the direction of their choosing.

At ADSCP we believe a Job Coach/Employment Consultant is a Trainer, Teacher, Problem Solver, Advocate, Friend, Mentor, and Leader to the Client and should function as a support system.

  • All persons are capable of achieving the means to live a rich and full life.

Policy on Confidentiality

  • The disabilities and especially the private lives of the clients are to remain confidential at all times – they have a right to privacy just as we all do.
  • The only parties that will ever be made aware of our client’s needs and concerns are those parties having implied consent, those parties serving as their legal guardians, or relevant parties of ADS/Career Pathways .
  • Employers DO NOT have the right to know what a client’s disability is, only that they have limitations, and then, only if THESE LIMITATIONS might specifically impact their ability to perform the essential functions of the job.

    Individuals have the right to receive accurate, easily understood information, and have the right to be guaranteed the protection of the confidentiality of their relationship with their provider, except when laws or ethics dictate otherwise.

    Any disclosure to another party will be time limited and made with the full, written, and informed consent of the individuals.

    Individuals shall not be required to disclose confidential, privileged or other information other than that which is necessary to attain the goals of the IPS, or Individual Plan of Service.

    The clients and ADS/CP develop each IPS together and change its fundamentals when needed; keeping all counselors and/or support staff advised of the changes.

    Information technology will be used for transmission, storage, or data management only with methodologies that and assure the protection of the individual's privacy.

    Information will never be transferred, sold or otherwise utilized by Avenue D Services/Career Pathways.

               Participation in Service Decisions

    Consumers have the right and responsibility to fully participate in all decisions related to their health care.
    Consumers who are unable to fully participate in treatment decisions have the right to be represented by parents, guardians, family members, or other conservators.
